operational error
An occurrence attributable to an element of the air traffic control system that results in less than the applicable separation minimums between two or more aircraft or between an aircraft and terrain or obstacles as required. Obstacles include vehicles, equipment, or personnel on runways or aircraft landing or departing on a runway closed to aircraft operations after receiving air traffic authorization.
